Bariatric surgery, option to obtain remission of diabetes

Mexico City. Bariatric surgery is no longer an aesthetic procedure, as was the goal in its beginning more than 40 years ago and is now an option for the medical treatment of obesity. And in the diabetic patient with this degree of overweight, the results can mean more years of life.

"In patients with diabetes with less than five years of disease evolution, with moderate obesity and with little oral medication, bariatric surgery reaches the remission of the disease between 85 and 90 percent of the cases," explains the doctor. Samuel Ordóñez Ortega, specialist supported by the Mexican College of Surgery for Obesity and Metabolic Diseases.

The bariatric doctor clarifies that the remission is not the same as the cure for diabetes. "The referral states that after the intervention, the patient spends one year without medication for glycemic control. Of course, in order to achieve this, you need your commitment and a multidisciplinary approach that includes a special diet and many accurate care. this is achieved in nine out of ten cases. "

The so-called bariatric or metabolic surgery may include two surgical techniques. One is the gastric sleeve, in which the stomach is reduced in its capacity from 500 to 600 milliliters, in its normal condition, to one hundred. The organ acquires a tubular form and reduces 80% of its gastric capacity.

The other procedure is the gastric bypass, where the stomach is reduced using clamps and through a bridge it attaches to the small intestine, so that the food avoids the gastric zone. It is the technique with the best results in the world in the last 20 years, according to the surgeon Ordóñez Ortega.

"Bariatric surgery achieves a glycemic equity, that is, it keeps sugar stable without the need for medication and gives the diabetic patient another 15 years of life," says the specialist assigned to the Clinic of Bariatric and Metabolic Surgery.

Bariatric surgery is suggested in patients with a Body Mass Index (BMI) of less than 35, and in whom the goal is the remission of diabetes. The index is the value resulting from dividing the weight in kilograms, between the square of the height in meters, to obtain Kg / m2.

When BMI is between 18 and 25, normal parameters are spoken, above 25 to 30 a person is overweight, between 30 and 34.9 degree of obesity 1, between 35 and 39.9 degree 2 and above 40 is of type 3

Finally, Dr. Ordonez Ortega warns of the risk that the interested party may place in the hands of those who do not know well the surgical technique. He says that in Mexico there is no official record of the number of metabolic interventions performed annually, but more than 10,000 surgeries are estimated without official endorsement.

"There are only 140 bariatric surgeons certified in a country of 120 million people, and seven out of 10 are overweight or obese. There is little information circulating in the media about what bariatric surgery can do for those living with obesity and diabetes. However, intervention is a tool for the patient and we teach him to get the best results. "
